A Clinical Study of Dupuytren's Disease in Korea


Abstract
Dupuytren's disease is one of the fibroproliferative disease in hand primarily, but same pathologic process can be seen in the subcutaneous tissue on the dorsum of finger(knuckle pads). Plantar surface of feet(planter fibromatosis) and in the
penis(Peyronie's disease). This disease is common in caucasian, especially in north europian, and epidermiolgy and the results after treatment are well known but in black and oriental race, it is rare and few reports were presented. So
epidermiology and
process of the disease are not established but just suspected to be similar that of caucasian.
Until now, 56 hands of Dupuytren disease in 41 men, including our 5 hands in 4 men, were reported in Korea and we analysed in the repect of sex, age of patient and age of onset, associated illness, involved hand and fingers, involved joint,
duration of
disease, degree of disease, occupation, operation method. Result after operation, and complication and then these were compared with foreign reports.
The results are as follows; 1) the mean age of onset is 49.7 years old 2) male : female sex ratio is 39:2 3) associated illness are 6 chronic alcoholism, 3 diabetes mellitus, 4 ectopic disease(2 plantar fibromatosis, 2 knuckle pad), 2 pulmonary
tuberculosis, 1 heart failure, 1 rhumatoid arthritis, 1 circulatory disturbance of affected hand, 3 trauma history, 1 family history 4) the mean duration of disease is 6.0 years 5) the ratio of hand involved is right : left : both hands=18:8:15

side fingers are frequently involved(4th; 54.7%, 5th: 33.3%) 7) single MP joint contracture is the most frequent in contracted fingers(42.7%) 8) most patient shows contracture(76.8%) and some show contracture without nodule(16.1%) 9) grade of

Meyerding classification is 0; 21.4%, I; 46.4%, II; 17.9%, III: 14.3% 10) Dupuytren disease seems more common in manual workers(66.7%) 11) regional fasiectomy was most frequently used as treatment method(76.6%) 12) the results after operation are

above good by Honner's classifiction(above good; 84.1%) 13) complications are 2 extension, 1 hematoma, 1 nerve injury, 1 infection with focal skin necrosis.
